From a Mac and/or Windows - 

1. Go to the Carleton https://onbaseweb.carleton.edu
2. Scroll the list of forms to find, and click on, the link titled "Stipend Request Form" in the Payroll Forms section
3. Leaving the top option set to the default value of "ADS," enter your Carleton User Name and Password

4. Complete all fields on the form to the best of your ability and click Submit at the bottom-left area of the form
